RULES AND REGULATIONS

For Grave Lots

I am a member of the Roman Catholic Church as a parishioner of St. Stephen's Knanaya Catholic Forane Church.

I, [BUYER], having arranged to purchase a Right of Burial Privilege in Queen of Peace Cemetery - Section - , Range - , Plot - #, Location Type – Single 1x34 wish to state that all of the following has been fully explained to me and I agree to be bound thereby.

1. The Cemetery is intended for all baptized members of the Roman Catholic Church since Catholics are strongly urged to be buried in a Catholic Cemetery. When a Catholic marries a non-Catholic, the Church does not wish to separate in death those who were united in life. Therefore, the Catholic’s immediate family members who are non-Catholic may be interred in this lot. These immediate family members include spouse, children, parents, grandparents, brothers and sisters, aunts and uncles, and grandchildren. This Privilege of Burial may not be extended to any deceased person who shall have departed this life not in communion with the Roman Catholic Church. The cemetery management may require Priest Lines to certify a person’s eligibility for Christian Burial.

2. The purchase of a Privilege of Burial does not confer in any way ownership of the lot. Ownership remains in Catholic Cemeteries of Long Island; my right is to use the above-mentioned lot for burials only as per the Privilege of Burial rules and regulations.

3. The Privilege of Burial is not transferable, and the determination of successors will be made according to Cemetery rules and regulations unless so directed otherwise by me in writing and approved by cemetery authorities.

4. The purchase price does not include the interment fee, foundation fee, or inscription fee (unless so noted in the sales contract) which must be paid at the time services are rendered. All such work will be performed by Cemetery personnel or designees.

5. Stonework specifications: To be provided by cemetery authorities at the time of monument application.

6. Refunds will be made for valid reasons only based on the purchase price of the lot less an administration expense fee that Catholic Cemeteries of Long Island will retain to cover administrative expenses incurred. Fees range from a nominal charge up to 10% of the purchase price.

7. On multiple grave lots appearing on one certificate, certificate holders are encouraged to complete an affidavit of designation specifying the persons to be buried within the spaces. For lots without designation, at the time of a burial order, the cemetery will provide a sketch of the lot layout for specific selection to open the specified place for burial.

8. The Permanent Maintenance (care) fee paid at the time of purchasing the Privilege of Burial includes leveling of the burial site, fertilizing, seeding, and mowing of the lawn area only. Private plantings, when allowed (see below), and the care of same are the responsibility of the certificate holder.

9. Plantings:
Monument Lots: Only annual flowers may be planted within a semi-circle of 18” in front of the monument (and the rear for four-grave lots and larger). No shrubs, bushes, vines, climbers, hedges, or growing boundaries are permitted. Artificial flowers may only be placed during the winter months (November 1st until one week after Easter).

10. Interments:
Three interments are permitted in a single grave if casket only or if wooden/tri-metal outside case is used.
Only two interments are permitted if a concrete vault is used.
Caskets: The maximum casket/box size permitted (including handles and hardware) is 26” High x 32” Wide x 88” Long.
Cremated Remains: In addition to the above-mentioned burials, up to three cremated remains may be buried subject to available and future inscription space on the family memorial. Cremated remains are placed in a cremation vault provided by the cemetery and included in the inurnment fee. All cremation urns/containers shall not exceed 12.5” Long x 6” High x 7.5” Wide.

11. Approved Cemetery Monuments/Markers or Bouquet Holders are the only grave markers permitted. Owners are asked to limit bouquet holders to no more than two per lot since each bouquet holder reduces the area reserved for planting. In Flush Marker Lots/Sections, the bouquet holder is part of the marker and additional bouquet holders are not permitted in the ground area.

12. Cemetery personnel take reasonable care to safeguard all monuments, bouquet holders and plantings. Nevertheless, due to the great number of cemetery visitors, it is impossible to guarantee any item from theft, damage, or loss.

13. Glass jars, statues, shells, vigil lights, marble chips and bric-a-brac of any kind are not permitted. Any articles of this nature placed in violation of the rules will be removed and disposed of without notice or compensation.

14. A complete copy of the current and dated Rules and Regulations document and current price lists will be available for inspection at Catholic Cemeteries of Long Island’s offices located in each cemetery during regular office hours. This copy will be available to the certificate holder and appropriate contractors/vendors as well as the general public. These rules are subject to change without notice for the overall beauty of the Cemetery or any other reason that cemetery authorities deem necessary for the good of the majority and the proper maintenance of the Cemetery. 15. Certificate holders and their heirs should report a Change of Address to cemetery authorities. The cemetery will assume that the last address on file is the current address.

Fully understanding the above, it is my wish to purchase a Privilege of Burial in the above-mentioned lot.
